A posterior talar process fracture is an uncommon injury and the literature on the subject is limited as a result. However, it is apparent that closed treatment of these injuries is generally associated with worse outcomes when compared to operative treatment due to associated sequelae. Surgical options include excision of the fragment, as well as ORIF.
